Career evidence templates
Five plain-text templates you can copy into any notes app: a weekly log, a self-assessment, a promotion case, a pay rise email and a CV line. Each one uses the same structure, so evidence you capture weekly drops straight into the others.
Weekly work log
Ten minutes, same slot every week. Most weeks you will fill three lines. That is enough.
Week of: ___ What changed because of me: What I owned (not helped with): Outcome, even a small one: New responsibility: Feedback, and from whom: Where the proof lives:
Performance review self-assessment
Three to five achievements, each written as Context, Action, Impact, Proof. Then one honest development area.
Achievement 1 Context: ___ Action: I ___ Impact: ___ changed from ___ to ___ Proof: ___ Where I grew this year: ___ What I want to develop next, and the support I need: ___
Promotion case
Show you already operate at the next level. Map evidence to the criteria, not to effort.
Role I am asking for: ___ Next-level criteria (from the framework or job description): Criterion: ___ | My evidence: ___ | Proof: ___ Criterion: ___ | My evidence: ___ | Proof: ___ Scope I now hold that my current role does not include: ___ Gaps I know about, and my plan for them: ___ What I am asking for, and by when: ___
Pay rise request email
Ask for a meeting first. Keep the email short and let the evidence do the work in the room.
Subject: Time to discuss my role and pay Hi ___, Over the past ___ months my role has grown to include ___ and ___. The results include ___ (recorded in ___). I would like to discuss my salary in light of this. Could we find 30 minutes in the next two weeks? Thanks, ___
CV achievement line
One line: an action verb, what you did, what changed. Keep the context and proof for the interview.
[Verb] [what you did] [for whom or where], [result as a before and after or a number you can prove]. Example structure: Rewrote the new starter setup guide, cutting time to first delivery from three weeks to eight days.
Why they share one structure
Every template is built on Context, Action, Impact, Proof. Capture it once in the weekly log and you never have to reconstruct it for a review, promotion case or CV.
You did the work. Keep the evidence. LAYG turns a 90-second weekly check-in into a dated record you can use for all of this.
